Evidence Based Treatment Modality For Edentulous Patients

Edentulism, defined as having no teeth, is usually the result of dental caries and periodontal disease. Edentulism has negative effects on oral function, psycho-socia lcondition and life quality of the individuals.
Considering, evidence based dentistry and depending on edentulous patient’s adaptive capacity, dentures can be classified as: conventional complete denture, implant-supported removable denture or implant-supported fixeddenture.
Implant-supported fixed denture has disadvanges as: loss of soft and hard tissue may be more difficult to compansate, more risk associaton to the patient and tissues, more expensive treatment modality and need more careful oral hygiene habit.
Conventional complete dentures rely on the residual alveolar ridge and mucosa for support and retention. Many patients have problems adapting to heir, especially mandibular complete dentures because of lack of support, lack of stability and lack of retention.
Generally, 1-implant  retained overdenture, 2-implant retained overdenture and 4-implant retained fixed prosthesis (all-on four concept) well accepted treatment options.
The use of 1- or more-implant, transforms complete denture to implant-retained overdenture, and enables the advantages as: prevents denture movement, improves denture stability, improves function and beter masticatory ability and reduced residual ridge resorption
Using two or more implants, either implant-supported removable denture (overdenture) or implant-retained fixed denture can solve problems related with conventional complete dentures. Advantages of implant-supported overdentures over implant-supported fixed prosthesis: the need for fewer implants, reduced cost, more straight-forward both clinical and laboratory procedures and easier access for oral hygiene.
The aim of this presentation will to discuss two-implant supported overdentures and all-on-four treatment protocol by means of patient age, cost, maintenance and patient satisfaction.

*Tretament of edentulous patients by using 2-implant supported mandibular overdenture opposed to complete denture is effective as all-on-four implant supported fixed prosthesis.
*The use of 2-implant to support a mandibula roverdenture is a reliable solution to solve problems that are associated with mandibular complete denture, especially on elderly patients.


Primary and secondary school education completed at TED Ankara Koleji. Graduated University of Ankara, Faculty of Dentistry in 1985. Obtained PhD degree titled “Some Mechanical Properties of Resin-Based Dental Materials” at University of Birmingham, School of Dentistry,  England in 1992. Achieved Associate Professorship and Professorship in 1995 and 2000 University of Ankara, Faculty of Dentistry, Department of Prosthodontics, respectively. The Dean of Dental Faculty, at Süleyman Demirel University, Isparta during 2000-2004. Had been NewYork University-USA, Toronto University-Canada, Tokyo University-Japan and several times Turku University-Finland, and TCBC-Turku-Finland as a visiting researcher. Now, working as a full time professor at University of Ankara, Faculty of Dentistry and head of Department of Prosthodontics. Has published 51 and 26 articles, national and international dental journals, respectively. Has presented 254 oral and/or poster presentations national and/or international dental conferences or congress. Invited speaker at 55 different national and/or international dental conferences or congress. Married and has one daughter. Both his wife and his daughter are dentist as well.
