Team approach in sleep apnoea


Obstructive sleep apnoea-hypopnoea OSAH involves factors that relate to the anatomical dimensions / resistance / muscle activity during sleep of the upper airways. So a  team approach to the treatment has to be taken into account as it is related to general health conditions, both as onset and as a consequence / complications. In particular a team approach is needed in sleep-disordered breathing in children, where apnea hypopnea index  AHI shows greater benefits by means of continuous positive airway pressure CPAP, still the gold standard treatment, but the number of hours is significant and might impinge on quality of life and reduce compliance to subsequent needed treatments. Nasal deformities have been also postulated, research is going on to show different aspects of possible interactions.

In adults orthodontists are required to consider that driving licences may be issued to applicants or drivers with moderate or severe obstructive sleep apnoea syndrome, who show adequate control of their condition and compliance with appropriate treatment and improvement of sleepiness, if any, confirmed by authorised medical opinion. So the role of the orthodontist in a team where comorbidity, compliance to treatment and results stability are part of an holistic approach should take into account also the new computer aided design CAD evaluation of pre / post surgical volumes in jaw surgical advancement.
